Assessment Checklist

  •  Identify:
    • Which assessment and subtests to use for target student (needs to be age/grade and domain appropriate).
    • Which norm equivalent you will use: age or grade.
    • Which recording form to use for target student.
    • All needed materials are available and in working order (i.e., audio-visual materials, computer scoring programs, extra recording forms and materials).
  • Inform:
    • Parents and student of date, time, location, and choice of assessment.
    • General education teacher of date, time, location, and choice of assessment.
  •  Carefully read basal and ceiling rules for each subtest you will administer.
  • Set up testing area
    • Area is free from visual distractions.
    • Area is free from auditory distractions.
    • Area is free from environmental distractions (i.e., sharp lighting, uncomfortable room temperature, unstable desk or chair).
  • Set up test
    • Easel can be clearly seen by student.
    • Both sides of easel can be easily seen by administrator.
    • Check for and listen to/watch any required audio-visual test materials
  • Review test form for:
    • Where to mark correct and incorrect responses.
    • Additional scoring requirements (i.e., accent and pronunciation marks, spelling).
  • Review:
    • Check manual again for testing guidelines and suggestions.
    • Check subtests and forms for age/grade and domain appropriateness.
    • Check all steps for completeness and accuracy.
